Have a client who has a clinic and is a trained nurse and specialises in Cosmetic Aesthetic treatments but can also do blood tests and IV drip vitamin treatments. She wants these all under different Google listings and potentially all different brand names, with each one featuring the service in the business name so it will naturally rank well, e.g blood tests near me...

Is this a good idea? Would there be any penalty to having so many businesses at the same address (She's talking about having 3 or 4 listings)

The difference in phone number means it's unlikely to get merged automatically (as long as they stay different). As discussed in other threads, the difficulty may be to prove that her new desired GBPs are indeed fully distinct businesses with video validation (which I'm going to bet was not very prominent when she did the original profiles).
I've just realised on further review she actually already has 3 different businesses running at the location and they all have individual GBP's... (it's quite a big building) the address is the same but the name and phone number are different for each, and all in different categories... this technique seems to have been successful for her... so it's quite hard to talk her out of going ahead with the new listings... reason she wants us is to further increase visiblity with SEO and for us to increase engagement/conversion by posting Google updates + help with review generation.

So we are looking at proceeding with this strategy, although we'll be strong on recommending she registers each business seperately as she has done previously and business details are the same as they will appear on Google

Ok good point, thanks for your input, I haven't actually done a video verification yet... We do have one client who needs to do one who is ignoring our messages!

She does have signs for each different business there outside the premises... so hopefully that would pass.. I'll remind her she'll need to get new signs made up for the new businesses as well!
